Primary Care Access Scheme

Background

  • The primary care access scheme was developed as part of a process to

review a specific type of GP contract called a Personal Medical Services (PMS) contract. This is a locally agreed contract as opposed to the national General Medical services (GMS) contract. There are 22 PMS practices, and 19 GMS practices in Waltham Forest.

  • They key principle for the review was to ensure all practices offered the

same access to patients for the same price. Prior to the review this varied significantly between PMS and GMS practices.

  • The scheme enables practices to receive additional payments for

delivery of locally developed indicators which are over and above the requirements of the core contract.

  • Access and patient satisfaction were the areas chosen because we

know that Waltham Forest patients have experienced some difficulty getting an appointment at their GP practice and we believe this scheme will improve this situation.

The Review Process

  • The CCG conducted lengthy negotiations with a group that consisted of

PMS, GMS and Local Medical Committee (LMC) representatives

  • The review meant that some PMS practices had their contract value

reduced whilst some PMS practices and all GMS practices could earn additional income through the scheme.

  • Following conclusion of the negotiations the proposal was formally signed
  • ff by the CCGs primary care commissioning committee and assured by

NHS England and Londonwide LMC

  • GMS practices could sign up to the scheme with effect from 1 July

2017

  • All PMS practices have now agreed to the new deal and the

scheme went live for them from 1 January.

  • The detail of the scheme is provided in appendix 1

What Are We Doing

As a part of the NHS England General Practice Forward View there has been funding released for each CCG across England. The CCG will look to procure a system/tool that can enable online consultations to take place within a General Practice setting. Research has shown that around 10% of patients will utilise this solution so this is unlikely to be something that will radically change the way a practice operates. This is intended to give those patients willing to use technology to make use of another channel to access primary care for routine purposes.

What Are We Doing

NHS England have given each CCG a deadline of 31 March 2018 to have spent the allocated funding or have committed contractually to a supplier. Where we are: 1. A review of the market has taken place at North East London (NEL) level. 2. Each CCG across NEL has taken part in joint working to look at one common solution for all practices across the 7 CCG areas in NEL. 3. Understanding what technical interfaces we need with our clinical systems to enable this to truly work and give benefit to patients and practices.

Remuneration committee Setting the pay for CCG executives and lay members. Advising on any payment issues that sets the CCGs at risk. Agreeing the new salary for the new accountable officer Lay member for NE London Individual Funding Request Panel This is a clinical panel which can compel local CCGs to fund treatments outside NHS tariff/NICE guidance. We look at a number of criteria. Recent examples have included a young woman who wanted breast reduction surgery due to muscular skeletal issues, and someone who wanted cosmetic surgery following obesity reduction surgery. Based on a presentation from an expert and evaluation against criteria - the panel must decide whether treatment should be funded.

  • Procurement process for NHS 111; attending a number of training meetings as a

patient representative, some online work (shortlisting applicants) and then a whole day

  • f in the assessment.
  • Promoting surveys and information; to community groups in my local area and

Residents Association.

  • Enjoyed commenting on patient leaflets etc. and have done this when the opportunity

has arisen.

  • For 2018, I would love to be more involved in longer lasting projects focusing on

specific aspects of health and patient experience. Cancer / geriatric medicine / end of life care / mental health.

  • Attended meetings; Patient Voice, launch of the blood testing
