  • What is it?

– A patient care reference tool for NAs and PCTs

  • Looks like a census report, with detailed patient care tasks added

– Pulls data from eRecord fields – Includes a variety of patient care tasks and directives relevant to NA and PCT role for each patient in their assignment – Is accessible via the Explorer Menu in eRecord – Is customizable......has numerous care categories that can be selected or deselected – Most importantly, IT SAVES THE NA/PCT STAFF 20-30 MINUTES EACH SHIFT!

  • Previously, this information was acquired by reviewing

Caredexes in eRecord or in verbal reports

  • Make sure all PCT’s/NA’s on your unit know how to access

and use this document

  • Work with staff to incorporate into their routine, including:

– Streamlining verbal report – Designing process for them to get the verbal info they still need – Spot checking to make sure it is happening the way you want it to – during the “learning curve”, you may want to have them turn their papers in so you can see these blanks are filled out appropriately

  • Because the report is customizable, you may want to

specify for your staff the minimum requirements as to which categories need printed.

  • Consider working with your nurses to encourage them to

“clean up” orders that are no longer relevant but need to be completed in E-record. Otherwise this info will continue to print on the list.

  • Think… do you have any current practices (like a CBS

clipboard where nurses write which patients need blood sugars for the NA’s/PCT’s) that may no longer be needed?

  • Use this info to help streamline your verbal report – focus your verbal

report on finding out info that is not in E-record. Blanks are provided for you on the report for some of this info.

  • Reprint the list periodically throughout the day.
  • For your information… this is version 1, version 2 will be coming out

soon.
