- FinalForms is a MANDATORY online system that will allow you to
register your student - athlete and complete most required forms electronically, saving you time and less paperwork. The only form that is still required to be handed in is the actual OHSAA Physical Form that the doctor will need to sign.

- 1. Go to https://brunswick-oh.finalforms.com
- 2. Click NEW ACCOUNT under the Parent Icon
- 3. Type your NAME, DATE OF BIRTH and EMAIL, then click REGISTER
- 4. Check your Email for a FinalForms Email, and click CONFIRM YOUR
ACCOUNT in the email text
- 5. Create your new FinalForms password and click CONFIRM ACCOUNT
- 6. Click REGISTER STUDENT for your first child

- Before the season’s first practice (or prior to your first
participation should you join the team after the season has started), you must have had a physical examination within the past year AND an examination clearance form must be on file at the school.
- Physical examinations are valid for 13 months from the date of
the exam. Exams taking place from May 1 to June 1 are valid for
- ne year plus through the end of the next school year’s spring

- Turn into your respective Middle School Building and turn
into the building secretary
- Also can be turned in at High School Athletic Office
- There is a blue drop-box outside of the athletic office for
after hours.
- Must be turned in before tryouts.
- A student athlete who shows to the first day of
practice/tryouts with their physical is not guaranteed the ability to participate that day
SLIDE 26 Participation Fees
- $80.00 per sport
- $360.00 max per family
- Federal Reduced Lunch Program = 50% fee (limited to one sport per student)
- Federal Free Lunch Program = no fee (limited to one sport per student)
- All participation fees are due by August 20th
- Two payment option is available if necessary. Please contact the school office
to arrange.

SLIDE 27 Transportation
- Student are required to be transported to and from away contests by BCSD
transportation.
- In the event of an extenuating circumstance, the student-athlete may be
transported by only his/her parent/guardian (Examples: SAT/ACT, another school extracurricular function, special family gathering). This should not be used just as a matter of convenience.
- Parents must submit in writing to the middle school athletic department with 24
hours notice the date which they will need to transport their student-athlete, which parent/guardian will be transporting the student-athlete, and a reason. Parents must sign and date.
SLIDE 28 Eligibility
- Passed a minimum of 5 courses the previous grading period.
○ Fall Sports (4th qtr., previous year) ○ Winter Sports (1st qtr. & 2nd qtr.) ○ Spring Sports (3rd qtr.)
○ 7 & 8 Grade (below a 1.8 GPA)
- Check your student's grades frequently in Progressbook & email

- Policy for in-eligible athletes is located in the code of conduct
SLIDE 29 Playing multiple sports same season
- You are allowed to play two sports offered during the same season for
fall, winter and spring. Example: being on the wrestling team while also being on the basketball team. Both these sports are offered in the same season (winter sports season) so it is allowed.
- However, you will be ineligible if you are a member of a school team
competing on a non-school team in the same sport during your school team’s season. Example: AAU Club basketball team during school’s basketball season

- Most sports teams typically practice right after school
- Wrestling and Basketball have some late night practices
due to space availability
