System Change

Current System: “Should we accept this household into

  • ur program?”

Future System: “What housing /service assistance is best for each household and quickly ends their housing crisis permanently?”

  • Program-Centric
  • First come first serve
  • Unique forms and assessment

processes to each organization

  • Disorganized referral process that does

not necessarily ensure match between need and service received

  • Uneven knowledge about available

housing and service interventions in the CoC

  • Client-Centric and System Driven
  • Prioritize the most acute
  • Standard forms and assessment

processes used by every program for every client

  • Coordinated referral process that

ensures a match between need and service received

  • Accessible information about available

housing and service interventions in the CoC

  • Collection of meaningful data

Next Steps

  • Hand over responsibility of the local implementation to

the local homeless coalitions (LHCs)

  • Provide guidance for tough conversations, e.g.,

eligibility requirements and closing the side doors

  • Analyze assessment data and present to Entry Points

and LHC representatives weekly

  • Work with HMIS staff to develop priority housing list and
  • ther functions
  • Development of a toolkit

What I’ve Learned…

  • It is hard work.
  • Things change all the time.
  • Everyone approaches this process in different ways.
  • Don’t try to reinvent the wheel!
  • You always need to explain what Coordinated

Access is.

  • CoC Written Standards are important.
  • Diversion is awesome!
  • Use your data.
  • Engage HMIS early and often.
