WHO IS SYDNEY NORTH PRIMARY HEALTH NETWORK? • Sydney North Primary Health Network is one of 31 Primary Health Networks established nationally by the Australian Government. • We are locally situated (in St Leonards), and here to improve medical and health services for patients, and in particular people who may be at risk of poor health. • We also need to improve how care is coordinated so that patients receive the right care in the right place at the right time. We work with patients, General Practitioners, Allied Health • providers, local health services and hospitals to improve the primary healthcare system.

AUSTRALIAN IMMUNISATION REGISTER (AIR) A national register that records vaccinations given to people of all ‒ ages in Australia The National Immunisation Program Schedule on the Department of ‒ Health Immunise Australia Program website specifies what ages your child should receive certain vaccines Parent’s eligibility to receive family assistance payments such as ‒ Family Tax Benefit Part A Supplement, Child Care Benefit & Child Care Rebate is linked to their child’s immunisation status Vaccinations can be given by your GP, nurse, immunisation ‒ clinic and some hospitals

  13. REFERRING TO SPECIALISTS • Work in both Public and Private hospitals • You will need a referral from your GP to see a specialist Without a referral you will not be eligible to receive money • back from Medicare • They focus on one part of the body or body system

REFERRING TO ALLIED HEALTH • Referrals to private allied health such as physio, podiatry, chiropractors do not require a referral, but you will not receive a rebate from Medicare, only private health insurance • In some certain circumstances they can be used to treat you as part of your GP management plan and you may receive a rebate from Medicare

  15. PRIVATE HEALTH INSURANCE Who is covered? Single, couple, family, single parent family What is covered? Hospital – covers your costs as a private patient in a public or private hospital Extras/Ancillary – dental, optical, physiotherapy, naturopathy, massage etc. Ambulance – can be purchased as stand alone cover. These can be purchased separately or combined. The extent of your cover depends on the policy you select.
