- Sydney North Primary Health Network is one of 31 Primary Health
Networks established nationally by the Australian Government.
- We are locally situated (in St Leonards), and here to improve
medical and health services for patients, and in particular people who may be at risk of poor health.
- We also need to improve how care is coordinated so that patients
receive the right care in the right place at the right time.
