Woodhouse Satellite Hub Evening and Weekend appointments are available with a GP or Practice Nurse at Woodhouse Satellite Clinic for patients registered with practices in the South East of Sheffield. Following a triage telephone call with our Duty Doctor, we can book people in for same day out of hours appointments. Please note:- If you are attending Woodhouse Satellite for a change of dressing you will need to take your dressing with you. There are also Physiotherapy initial assessment appointments available. New Starters The Practice would like to welcome Karen Goddard to the Team. Karen is an experienced Practice Nurse with many years experience. There are also many voluntary and direct access services that are not well known. We are, therefore, training our Receptionists to signpost you to the right services first time. Please do not be offended if they ask what the problem is when you call to make an appointment. They need this information to ensure you are seen by the most appropriate service/specialist. Our Receptionist will never offer clinical advice or triage, it is about offering you choice to see other specialists within our practice team. This will help free up GP time to care for the our patients with complex needs or serious health problems. More importantly it means you are seen by the clinician who is best placed to deal with your problem. Welcome to the first Hackenthorpe Medical Centre Newsletter . These will be issued quarterly with a view to keeping you upto date with changes/progress at the Practice. Flu Season 2017 As winter is approaching we are encouraging eligible patients to have their flu jab. To be eligible you must meet one or more of the following:-

  • Pregnant
  • Child or Adult with a long term condition (ie

heart or respiratory disease

  • Child or Adult with weakened immune system
  • Carer
  • Children born between 1st September 2013 – 31st

Sheffield NHS Walk In Centre

The walk in Centre is

365 days a year for minor illnesses and ailments. Broad Lane, Sheffield, S1 3PB (just off West Street) Bus 120 or Tram

A&E

A&E is another over stretched service - but valuable in very serious

  • r life threatening

situations. A&E is located at the Northern General Hospital

Telephone appointments are available with a GP if you need to speak to a Doctor but do not necessarily need to be seen. However, please only use this service after considering the above options (Self-help, NHS 111 or your Pharmacy).

Rebecca is our Advanced Nurse Practitioner at Hackenthorpe Medical Centre. She is a very experienced nurse who has undergone extensive and additional training, to enable her to treat common health problems in patients. Rebecca also works alongside the GPs to see some of the same-day urgent cases

Holiday Vaccinations

If you are travelling abroad and think you may need vaccinations please collect a ‘Holiday Vaccination Form’ from Reception. You will need one form for each person travelling. Once you have completed and returned to Surgery an appointment will be made for you to see the Practice Nurse to receive your vaccinations.

Reception Team

Our Reception Team are trained to do a difficult and demanding job. They are following the instructions of the doctors and procedures and protocols laid down by the Practice Please give them as much information as you can in order to help them to help you. They are bound by the same confidentiality as the doctors and nurses and anything you tell them will be treated in absolute confidence.

Long-Term Condition Annual Review

The Electronic Prescription Service is an NHS service. It gives you the chance to change how we send your prescription to the place you choose to get your medicines

Home Visit

Home visits may only be requested for a patient who is too ill to attend the

  • surgery. The Receptionist will ask for details to enable the

doctors to assess the degree of urgency when planning their rounds. Please make best use of the Doctor’s time by attending the surgery if at all possible Three patients could be seen in surgery while one patient is being seen at home

Fire Evacuation

In the unlikely event of a fire you will be asked by staff to vacate the building as safely and as quickly as possible. If the fire alarm sounds, your co-operation with staff instructions is essential to a swift and safe evacuation of the premises. The assembly point is located outside Lo’s Pharmacy.

Telephone Consultations

Sometimes a face to face consultation may not be necessary, eg medical advice, minor ailments, certificates or medication queries. For these problems you may telephone the surgery and request a telephone consultation with a doctor or nurse, who will normally phone you at the end of their morning clinic.

Prescriptions

Please allow at least two full working days for prescriptions to be processed. Prescriptions will be processed twice per day. (10 am and 3pm) Any requests for urgent prescriptions should be handed in at Reception

Well Baby Clinic

This clinic is held every Tuesday

1.00 pm – 2.00pm

(Pre-bookable appointments only) The Doctor together with Practice Nurse are in attendance, offering child health guidance and welfare, child immunisations and child development and surveillance. Community based clinics are available for the Health Visitors (please see next slide, or ask at Reception for more details).
