This description and name should indicate why the registry was created; for our example, we want to include the Women’s Rising Risk grant name in the title and description. 4. Select Inclusion and Exclusion Criteria:
– Over 18 – Female – Inclusion: Dx of Depression and Hypertension – Exclusion: Active Pregnancy – Remember to select an operator.

– Displays a single number. You can display any of the following measure components using a counter: numerator, denominator, result, gap (number of patients who do not meet the measure criteria) or to target (number of patients to meet the set target). Counters can help set context for other performance widgets you also plan to include in your dashboard
– Illustrates the result of a performance measure as a percentage in a dial format, with targets (if set). The performance line going around the dial changes colors as the performance reaches targets. This widget gives you a quick sense of current period performance.
Widget Types
– Demonstrates a measure result as a trend line with reporting periods on the -axis. If targets are set, they are displayed as green (primary) and yellow (secondary) lines. This widget is useful for providing context and understanding performance
volatile, trending up, or trending down.
– Displays multiple measures which share the exact same denominator in a small scorecard format. Scorecard shows the number of patients in the shared denominator, as well as the name, numerator, result as a percentage, gaps, to target and target stoplight indicator for each measure on separate rows. The measure family widget is a powerful tool for showing movement within a patient population, however the widget is

– Displays the results of a measure grouped by any available grouping option. For example, the table might show the A1c >9
factors contributing to the result, and indicators for possible improvement strategies.
– Displays the selected center, location, provider, etc. performance percentage with targets and stoplight target coloring to indicate if the target was not met, secondary target was met, or primary target was met/surpassed. The second line indicates the average performance for the measure to help users understand how they compare with peers, and what their performance means. Use this widget to quickly gauge which measures need the most focus.

1. Go to Admin > Report Configuration > Dashboard Administration and click New Dashboard. 2. Create a name and description for your dashboard to indicate its purpose; in this case, you could name your dashboard HRSA Diabetes Improvement Plan. 3. Uncheck Global Period Type. 4. Add a Counter widget using any diabetes measure and select the Denominator measure component to show the total number of patients with diabetes. Select the Period Type appropriate for the data you wish to see. 5. Add Percent of Total widget using the Patients with Face to Face Encounters measure and selecting
