Statement of attendance to Accident Clinic I confirm that I have today attended an accident clinic, due to me having an accident within the last period. We have discussed and I am fully aware of: Safe and unsafe behaviour The need to report hazards and unsafe acts committed by others The need to look after the safety of myself and others That nothing is so important it is worth putting ourselves at risk for The company does not expect us to take risks If I am unsure of what to do at any time I should ask .
