destroyed ed equipment equipment that is less than five (5) years old must be reported in writing State CTAE Director within ten days of the discovery of the loss.

  • Include the name of the specific item, the cause of

the loss (stolen, lost, damaged, destroyed, etc.) and the value of each item.

  • State CTAE Director will then contact GSFIC, who is the
  • wner of the equipment.
  • LEA is responsible for the replacement of the value of

the equipment lost. LEA has 60 days from the date the report was filed to replace the missing equipment.
