Human Error is something that has been done that was “not intended by the person who did it;” It’s not a conscious decision to depart from expected behavior. At-Risk Behavior is when an individual saw the risk and made the choice to do it anyway because the individual reasonably believed the risk was insignificant or justified; Assumed success probable.
