  • STRIDE referral received May 7, 2013 - Woman in her late 30’s, diagnosed with

schizoaffective disorder. Articulate, experienced in child care and cleaning. Had not worked in 3 years. Client identified her employment barriers as the following: large gap in work history and lack of updated resume.

  • Employment Specialist assisted with ODSP-ES application. ODSP-ES

authorization received May 22, 2013. Developed employment plan, re-designed résumé, created cover letters. ES coached and supported client to approach local

  • employers. After client secured competitive employment, ES and client stayed

in regular contact through phone communication and face to face meetings.

  • Outcome: client secured child minding position on July 5, 2013 at a local gym.

ODSP-ES placement was achieved in September 2013. Client remains in this position and is satisfied with current employment status. Client is in the retention phase and continues to receive support from ES.

  • ODSP-ES authorization received June 15, 2012. Man in his mid 50’s,

diagnosed with Anxiety Disorder and heart health issues (client had heart attack in 2011). Client was intelligent and had worked as a home renovator for several years. Client identified his employment barriers as the following: feelings of anxiety and an inability to work on physically demanding jobs.

  • Employment Specialist developed employment plan, created résumé and

cover letters. ES regularly assisted client with application process and approached several employers on client's behalf. Client required a high level

  • f counseling support to deal with his fear of job searching and feelings of

anxiety with regards to work. Client showed poor follow up on assigned tasks and self sabotaged at a number of interviews.

  • Outcome: Client did not secure a job. Due to client’s lack of follow up and his

lack of motivation to work, client was discharged from the Employment Connections program in October 2013.
