  • Reminder letter is faxed to office 15 days post

discharge.

  • Suspension letter is faxed 22 days post discharge.
  • Please make sure that the physician is given these

very important documents.

  • Also please encourage the physician to access his

incomplete records at least every 7 days.

  • In order to avoid suspension , physician must

complete signatures and dictation . ( this includes any delinquent charts of their PA, NP, Resident)

RADIOLOGY FAX #

The Radiology fax number for both campuses and all modalities is 796-5199.

If someone is asking you to fax them information to a different fax number in Radiology, please call Lashelle or Maria immediately. If we can figure out who and why they are asking you to fax directly to them, hopefully we can eliminate most of these duplicate faxes. Lashelle 725-4447 Maria 725-6598 We know that a call to us is more work for you, but we need a place to start.

asking for the same order.

It’s a one way street.

The schedulers (Terry, Mel, Sandra) who are the

  • nes we have assigned to the e-fax will call until

they have the order and will then forward it on to whoever needs it. In the mean time if Patty from C.T. called for the order and you fax it to her, she is happy and scans the patient. Meanwhile, the schedulers are still thinking that this patient is still in the exam room waiting and will continue to call even though the patient is done and gone.

Signatures

When your physician orders a Radiologic study, he is prescribing a study in which we will be administrating a dosage of radiation. You wouldn’t send that patient to the pharmacy with a prescription not signed because they would not accept it. We can not accept it either.

The Physician

A Nurse practitioner or Physician Assistant may sign in

  • rder to get the exam done. However, medical

records will be contacting the physician to co-sign later and your physician may end up on the delinquent list.

  • Program includes classes for diabetic patients and one
  • n one sessions with a dietician
  • Participants must be financially in need (200% or below the FPL)
  • Classes are offered in a 4 part course. If attendees

come to 3 of the 4 classes offered then they receive a free glucometer and can purchase discounted vouchers for strips (50 strips for $5 and 100 strips for $10)

  • Classes are currently offered year round
