Evaluation Criteria General Information (10 Points)  Design Experience of the Prime Firm (20 Points)  Construction Administration and Inspection (10 Points)  Experience of Key Personnel and Subconsultants (25 Points)  Project Understanding and Approach (30 Points)  Overall evaluation of the firm and its perceived ability to provide the  required services (5 Points) 5

  6. Project Description • The City has developed electrical, instrumentation and control standards for the entire Water Services Department which are used to provide consistency in the control systems. • The selected consultant will be enforcing these standards on all projects. • The areas of work are in the design phase, construction installation inspections, and commissioning testing and calibrations of the EIC systems. 6

  7. Scope of Work • The selected firm will provide: • Design support services consisting of presenting advice and technical consultations relative to installation techniques and procedures • Attend design meetings • Support the engineer with research of existing conditions • Support the engineering consultant with the understanding of the Water Services Departments standards for EIC systems • Provide construction and commissioning support services 7

Submittal Requirements:  Cover Letter – which includes full firm name, address, phone number, vendor number and email address for contact person  Evaluation Criteria – Address the SOQ evaluation criteria and include project organizational chart.  Additional Content – Resumes and other information may be included (within the permitted page limit) 8

Submittal Requirements:  Provide 7 copies of the State of Qualifications  Maximum 12 pages permitted to address all content  Paper size shall be 8 ½ x 11  Font size may not be less than 10 point  Each side of page containing evaluation criteria and additional content will be counted toward the maximum page limit noted above  Front/back covers, cover letter, Table of Content pages, and divider (tab) pages will NOT be counted toward the maximum page limit, unless they include evaluation criteria and additional content 9

  10. Grounds for Disqualification  Receipt of Bid after the specified cut-off date and time  Too few copies of the submittal  Deposit of Bid in the wrong location  Violating the “ Contact with City Employees ” policy 10

  11. Vendor Registration  To do business with the City you MUST register in procurePHX at https://www.phoenix.gov/procure.  Phoenix values its relationships with its vendors.  To become a vendor with the city, you MUST complete the vendor self-registration process.  The vendor number must be listed on the cover of the SOQ when submitted  It takes the city about two business days to approve the registration.  Part of the registration process is determining your taxing jurisdiction, your eligibility for small business programs, and verifying your submittal of a signed W-9 form from the IRS.  For questions about procurePHX , please send an email to vendor.support@phoenix.gov 11
