What is the Enhanced Care Register? • In December 2012, a programme of action asked that ‘all primary care trusts put together a list of all people with learning disabilities who have mental health conditions or behaviour that challenges in NHS-funded care.’ • This work was completed and NHS England are now calling for the list to include people from the whole population who have learning disabilities and Autism Spectrum Disorder in the local area. • This list of people is the enhanced care register. 1

Why do we have the Enhanced Care Register? • It aims to find adults and children (who are moving into adulthood), with a learning disability who are at risk of being admitted to a specialist hospital. • The choice about who will be put on the list, will be made locally, based on conversations between health and social care organisations who work closely together. • If it is agreed that a patient will be included on the list, then their permission to be included on the list must be recorded. 2

How often do we meet and who attends? • We meet once per month and review the list; adding patients or removing them depending on their needs. • The meeting is chaired by a Clinical Commissioning Group commissioner and is attended by senior representatives from health and social services. • Children's services also attend, to discuss cases which include children who may need to begin moving into adult services allowing us to plan for the future. 3

What is a Care and Treatment Review? • NHS England published (Oct 2015) a plan setting out hopes for putting into practice Care and Treatment Reviews for people with learning disabilities in England. • The document detailed how CCG commissioners will work together with partners to make sure individuals in the community who are at risk of admission into hospital are identified. 4

  6. When does a Care and Treatment Review happen? • A Care and Treatment Review happens when a patient is at risk of being admitted to hospital or is already admitted. It should happen within 3 weeks of admission and every 3 – 6 months once an individual has been admitted. • There are two types of Care and Treatment Reviews: - Community Care and Treatment Review - Ward Care and Treatment Review • Both follow the same or similar plan. Community Care and Treatment Reviews are arranged when the professionals treating or supporting the patient believe that one is needed because of risk of admission. 5

How is a Care and Treatment Review different from other meetings? • Who attends? • The Patient • 2 independent experts , chosen by NHS England • Patients support network – including; carers, support workers, family, social workers, nurses, psychiatrist, psychologist, Occupational Therapist. • The meeting is chaired by the Clinical Commissioning Group commissioner. The meeting can last all day but only the chair and independent experts may need to attend for whole time. 6

  8. How is a Care and Treatment Review different from other meetings? • They usually follow a set of key questions to make sure the patients needs are fully covered. • They listen to everyone's opinions and make recommendations to keep the patient well and safe in the community and ensuring their right are protected. 7
