25th February 2016

A worker carry out troubleshooting on their truck without stopping the truck’s engine. Unfortunately, while doing the troubleshooting, the truck suddenly moved back and crushed the worker’s head. The worker died at the incident location

Construction Site - Drowned

22nd February 2017

Two workers were installing the final foundation works for a proposed pedestrian bridge across Sungai Kedayan. They were both connected to the tip of the crane’s boom by safety harness when the crane toppled over and the crane boom fell into the river. Both workers fell into the river while still being attached to the crane’s boom. Both workers were brought back to land. Unfortunately, one worker died as he was unable to release his safety body harness from the submerged crane’s boom.

Private Residence Renovation Work (Tutong) – Fall From Height

12th April 2017

Workers carried out a renovation work (roofing and painting) at a private residence in Tutong area. One of the worker carry out his work on the roof, doing measurements at the edge of the roof on the front side of the house when he suddenly fall from the roof (approximately 6 meters) and landed on the cement floor on the ground level. His dead was due to cranio-cerebral injuries secondary to fall from height.

Fire at Launderette

26th July 2017

An explosion and flash fire occurred at the launderette. There were three persons in the launderette at the time

  • f incident. The Owner and one of the worker were trying to fix an LPG leak identified earlier in the same day

when the fire occurred. They were both seriously injured with burns (second degree more than 40%), the third person suffered a superficial smoke inhalation.

School Construction – Fall from Height

3rd August 2017

Workers were working on a newly installed glass roofing on one of school’s section. However, one of the section they working on was unfinished i.e. without glass roofing. The area was not cordoned. Workers were unaware of the unfinished section of the roof. One worker accidentally stepped on the unfinished area and fell from a height of approximately 15 feet and landed on concrete flooring. He was not wearing safety harness.

21st November 2017

Workers cutting coiled tension wire weighing around 3.5 tons. The coiled wire suddenly fell, pinning the victim to the ground, crushed the victim’s head to the ground.
