Adult Day Supports  Service remains available – w/modifications  Allow time to increase capacity of new, integrated services  Create clear expectations/communication/training/guidance around how long someone would be in service (i.e., would we expect someone to spend 5 days a week in this service? 40 hours? All 40 at the facility? Etc.?)  Consider provider capacity (i.e., should we certify any new providers?)  Require CBDD “prior auth ” initially, annually  Add Outcome statement  Outline Expected Activities  Service must comply w/HCBS rule  1:8 max  Continue to monitor use while new services are brought up/capacity is built EVERY PERSON. EVERY TALENT. EVERY OPPORTUNITY

Vocational Habilitation  Service remains available – w/modifications  Allow time to increase capacity of new, integrated services  Create clear expectations/communication/training/guidance around how long someone would be in service (i.e., would we expect someone to spend 5 days a week in this service? 40 hours? Integrated Prevocational Skill Building (Career Planning)  Add Outcome Statement  Outline Expected Activities  1:1  Everything up to employment (i.e., discovery, career exploration, job development, assistive technology assessment, benefits counseling, etc.) EVERY PERSON. EVERY TALENT. EVERY OPPORTUNITY

  10. Supported Employment – Small Group  Add Outcome Statement  Outline Expected Activities  1:8  Requirement of Minimum wage  Must be integrated  Intended to be transitional (consider CBDD pre-auth/reauth process) EVERY PERSON. EVERY TALENT. EVERY OPPORTUNITY

Supported Employment - Individual  Add Outcome Statement  Outline Expected Activities  1:1  Focus- On the job supports/services (also includes AT, benefits)  Reimbursement based on the following stages:  Initial/Int ntense(i.e., time-limited/not long-term supports to help learn/become stable on job)  Stabil bilization(i.e., regular contact w/person and/or employer)  Maintenance(i.e., 1x/month, on/off site, monthly payment, incentive to stay engaged/connected) EVERY PERSON. EVERY TALENT. EVERY OPPORTUNITY

  12. Non-Medical Transportation  Services delivered in facilities – to/from only  Integrated community services – incentive -- address per-trip vehicle and/or per-mile rate EVERY PERSON. EVERY TALENT. EVERY OPPORTUNITY
