Discussion Topics

 INDOT Quality Assurance Procedures/Scope  Focus Moving Forward/Reference Materials  Review of Changes to the Bridge Inspection Requirements

slide-3
SLIDE 3

Indiana Bridge Inspection QA/QC Procedures

Need for QA/QC Procedures?

 Federal requirement for QA/QC

♦ 23 CFR 650.313(g) Quality Control and Quality Assurance

requires each state to assure that systematic Quality Control (QC) and Quality Assurance (QA) procedures are being used to maintain a high degree of accuracy and consistency in the inspection program

 Without QA/QC procedures in place, FHWA would find INDOT non-compliant, NBIS Metric #20  INDOT designated Beam, Longest & Neff (BLN) as Quality Assurance Officer (2011 – Present)

slide-4
SLIDE 4

Indiana Bridge Inspection QA/QC Procedures

Goal of QA/QC Procedures?

 To Help Ensure:

♦ Accuracy ♦ Consistency ♦ A Quality Product ♦ Public Safety ♦ Maintenance and Repairs ♦ Prioritize Recommended Work ♦ Identify Training Needs

slide-5
SLIDE 5

Indiana Bridge Inspection QA/QC Procedures

QA/QC Procedures

 review is the Internal Review conducted from within an inspecting agency  Focuses on at-risk bridges  Office and field reviews  Reviewed by Inspection agency – INDOT or Inspection Consultant

slide-6

QA/QC Procedures

 review is the External Review conducted from outside an inspecting agency  Independent oversight reviews to focus on at-risk bridges  Reviews by INDOT and BLN

Quality Assurance Procedures

 Quality Assurance Reviews

♦ QA/QC Procedures – Part 2 Indiana Bridge Inspection Manual (IBIM) ♦ INDOT

  • Control Bridge – 2 year cycle

♦ BLN and BFS

  • Independent Oversight Review
  • Bridge File Review
  • County Load Rating Verification Reviews
slide-8
SLIDE 8

Indiana Bridge Inspection QA/QC Procedures

 Quality Assurance Review Scope

♦ All reviews and forms in accordance with QA procedures per IBIM Part 2 ♦ Bridge reviews as selected by INDOT ♦ 24 Bridges chosen for team leaders who did not attend control bridge, per IBIM Part 2, Section 3.2

 Changes in Scope

♦ QA form changes, IBIM Part 2, Appendix ♦ Office reviews revised to bridge file reviews ♦ Reviews based on Metric comparison and not a score

Independent Oversight Review

 Independent review of inspection data and findings for accuracy  Selected bridges based on posting, condition ratings, need for improvements, and Team Leader  Compared QA Officer results to

  • riginal inspection results and

notes  Reviews Conducted on District and Consultant Team Leaders throughout the state

slide-11
SLIDE 11

Indiana Bridge Inspection QA/QC Procedures

Bridge File Review

 Review of bridge files for accuracy and completeness  Selected bridges by INDOT search of files for known and suspected inaccuracies  Bridge file checked for required items as

  • utlined in the AASHTO Manual for Bridge

Evaluation, Section 2  Reviews of County and District bridges

slide-12
SLIDE 12

Load Rating Verification Review

 Independent Review of load rating computations for county bridges  Selected bridges based on INDOT query, consultant, team leader, condition ratings, and load posting  QA Officer performed field inspection and load rating  Compared QA Officer load rating to consultant load rating  Results reviewed based on QA form and NBIS Metric 13

slide-13
SLIDE 13

Indiana Bridge Inspection QA/QC Procedures

 Determined from findings from all QA reviews  Based on reoccurrence of similar comments and procedures

♦ Bridge File ♦ Critical Findings ♦ Load Rating ♦ Fracture Critical Reports

Focus Moving Forward:

Bridge File Documentation

 IBIM Part 1, Section 2.5 and 4.1  Official bridge file is the central database information in BIAS, used to report NBI data and supplemental state information to FHWA  Historic data and updated bridge inspection data  Other attachments, past reports, load rating summary, correspondence, Scour Plan of Action reports, and special reports such as Underwater, Fracture Critical, Special Detail

Indiana Bridge Inspection Application System (BIAS): https://indot-it.bentley.com/login.aspx

Bridge File Documentation / Reports

 Note enough deficiencies to provide justification for condition rating.  Verify deficiencies noted are consistent between sketches, comments, and executive summary  Bridge deck rating for adjacent box beam bridge without deck to be based on beam condition not wearing surface condition  Approve bridge data within 60 days of inspection  Use text and abbreviations that are easy to follow

slide-16
SLIDE 16

Indiana Bridge Inspection QA/QC Procedures

Critical Findings

 Structural or safety related deficiency that requires immediate follow-up inspection or action  IBIM Part 1, Section 5.2

♦ All bridges - Notification to State Program Manager of critical finding and document finding in BIAS, IBIM 5.2.2 ♦ Documented in BIAS under Maintenance Tab

slide-17
SLIDE 17

Indiana Bridge Inspection QA/QC Procedures

Critical Findings

 IBIM Part 1, Section 5.2

♦ Locally owned bridges – Report to ERC and State Bridge Inspection Program Manager ♦ Inspection agency to document action taken in BIAS ♦ Take action in reasonable time ♦ Provide closeout documentation in BIAS to closeout critical finding report

slide-18
SLIDE 18

Indiana Bridge Inspection QA/QC Procedures

Load Ratings

 IBIM Part 3  When in the Field……

♦ Take extra time in field to verify structure member sizes,

bridge dimensions, and section loss

♦ Document section loss and remaining section ♦ Verify condition at each phase of inspection, including

annual inspections

slide-19

Load Ratings

slide-20
SLIDE 20

Indiana Bridge Inspection QA/QC Procedures

Load Ratings

 When in the Office…..

♦ If significant deficiencies or change in condition visible, calculate rating quickly and recommend load posting changes ♦ Verify remaining section or section loss noted matches load rating calculation ♦ Include shear and moment in load rating calculations ♦ Verify load rating calculations or summary are signed and include Load Rating Engineer(LRE) information

slide-21
SLIDE 21

Indiana Bridge Inspection QA/QC Procedures

Load Ratings

 When in the Bridge File…

♦ Inspection agency to make sure load rating information

added to bridge file in BIAS, does not have to include all pages of the calculation

♦ Make sure load rating matches BIAS data, must

unapproved initial reports after load rating report approved

♦ Make recommendations for posting

changes if needed. Remember to recommend advance posting signs.

Fracture Critical Reports

 IBIM Part 4

♦ Develop Fracture Critical or Special Detail reports

♦ Providing fatigue categories and ratings for all

fracture critical members at connections

♦ Members within 12”

  • f connection

Fracture Critical Member (Tension) Limits of Fracture Critical Connection (12”)

Although this member is not Fracture Critical, this portion is within the limits of a fracture critical connection

Fracture Critical Member (Tension)

slide-23
SLIDE 23

Indiana Bridge Inspection QA/QC Procedures

Fracture Critical Reports

 IBIM Part 4

♦ Fracture Critical Reports to provide condition ratings and fatigue categories for floor beams at connections

♦ Need to report section loss ♦ Need to report cracks or holes
