  • Early Adopters (1997) in learning and implementing a robust HP

program

  • HP tools and concepts are applied daily and are fully linked to all

departmental and site processes

  • All employees are fully trained on the concepts and expectations
  • External agencies, INPO, USA, etc conduct assessments of the

plant’s performance in this area

  • There is a site manager with responsibilities for Operating

Experience (OE), HP, and Corrective Action processes. Staff of 6 for OE & HP work

FOSSIL GENERATION: (7 fossil units + Peakers, ~ 1,500 employees)

  • 2003 – All employees are trained in HP Fundamentals
  • 2005 – All employees receive HP refresher course
  • 2008-09 – A series of significant unwanted events occur

including a string of similar events related to high voltage switching.

  • 2009 -FG establishes a HP Champion to focus on Human

Performance at each plant. Each site has HP lead/team. Fleet HP Organization has ~ 30 volunteers.

Fossil Generation Continued

  • 2010 – metric employed quantifying # of HP events, Established

4 triggers to prompt full TWIN Analysis (Environmental Exceedences, Unit Derate, OSHA Recordable Injury, Cost to the budget from a single event in excess of $50,000

  • Targets and performance to date:

– <50 HP Resets…Actual: 68 – <175 HP Events… Actual: 168 – > 30 Fleet consecutive Reset Free Days…Actual 15

  • 2011-Goals: Review and realign metrics to stronger safety focus

and improved equipment reliability, develop a sustainability plan, Improve web site and information sharing, add volunteers on each site, improve employee HP knowledge, and more.

Summary of replies from Committee members: Why do people not follow known safety procedures?

  • Time and Comfort
  • Easier - following procedures can take longer and result in a shorter break time.
  • Because that’s the way we’ve always done it, as long as I’ve been here.
  • Peer-pressure, don’t want to look foolish in front of others.
  • Think they have a “better” way than the procedure
  • The supervisor really wants this done in a hurry
  • Perceived time restraints
  • Complacency
  • Laziness, especially when nobody is watching
  • Not focusing on the job, distracted by personal problems, co-workers, etc.
