ccountability
Principles
➢Clearly describe and get agreement on the goals and purposes o f the Fund
Governance
➢Accountability is to the ACH governance, not the organizat ion housing it. An MOU is one mechanism to operationalize the relationship
Implementation
➢Three ACHs have selected their Wellness Funds and b e g u n to identify funding sources
Wellness Fund
➢Development of the Sustainability Plan and Wellness Fund i san iterative process, in conjunction with the overall governance of the ACH and the Portfolio of Interventions. ➢Individual logic models are useful for identifying outcomes a n d benefits of the PoI and ACH—and making the case for investing ➢The Wellness Fund can also be a vehicle for braiding and a l i g n i n g funding and resources, even if it’s not happening explicitly through the Fund. ➢Early seed funding has come from the ACH grant, county f u n d i n g , a local foundation, and a health plan
