The Better Care Fund: What is it?

  • A national scheme intended to encourage health and social

care to work together more closely – as required by the 2014 Care Act.

  • 2 one year plans, 2015/16 and 2016/17.
  • The focus of Hillingdon’s Plans to now – Older People 65 +

population.

  • For 2015/16 and 2016/17 plans little new money, more

about doing things more efficiently.

  • Next step a two year plan for 2017/19. New money for

social care; position for health not announced yet.

Better Care Fund 2016/17 and Hospital Discharge

  • Achievements:

New patient information booklet. Integrated Discharge Team. Hospital Multi-disciplinary meetings (MDMs). Hospital Multi-disciplinary meetings (MDMs). Homesafe, Rapid Response and Reablement. New bed-based discharge to assess arrangements in local care homes. 7-day assessment arrangements by care homes to support hospital discharge.

Increased care planning in GP surgeries. Community multi-disciplinary teams (MDTs). H4All Wellbeing Service. A new single point of access for Carers. A new single point of access for Carers. Planning consent for Grassy Meadow Court and dementia resource centre. Introduction to Dementia training to staff working in GP surgeries as well as the Council's contact centre and libraries.

2 Care Connection Teams (CCTs) . An additional consultant geriatrician post that will help to support community health teams. 9 new Patient Flow Coordinators at Hillingdon Hospital.
