It is required that all students attending the Washington, D.C. trip meet certain academic, behavior and attendance standards in advance of the trip. Please read, initial by each requirement, and sign at the bottom of the page. Please return the contract to Mrs. Klock or Mrs. Pepe no later than Friday, October 19th. Initial here: ______ Participants must attend school regularly and on time. Excessive lateness and/or absences can warrant removal from the trip ______ Participants must exhibit model behavior at school and in our community by following our TUSKER guidelines as outlined in the SMS Student Handbook. Students, please be aware of these guidelines and monitor your actions as you progress through each school day. Parents, should the need arise, we will notify you if your child’s trip status is in jeopardy due to poor academics, attendance or behavior. ____________________________________________ Student name (please print) ____________________________________________ _______________________________ Student signature Date ____________________________________________ Parent/guardian name (please print) ____________________________________________ _______________________________ Parent/guardian signature Date
