Survey Administration • ODP requests randomly-drawn samples for each AE from the Home and Community Services Information System (HCSIS) and provides them to the AEs • AEs and IM4Q local programs eliminate names which do not meet the sampling criteria • Local programs contact the people included in the samples to ask if the person is willing to participate in an interview, and if so, to schedule it 8/3/2017 4

Support Families Throughout the Lifespan • Family Members Who Receive Information to Help Plan Services • 73-74% reported always or usually receiving information in the first two years, but increased to 79% in the third year • An average of 15% reported sometimes receiving information • 10% reported seldom or never receiving information in the first two years, but decreased to 7% in the third year 8/3/2017 7

Promote Health, Wellness, and Safety • Individuals and Self-Advocates Who Exercise monthly • An average of 36% reported that they who never go out for exercise • An average of 19% reported that they exercise once/week or less • An average 46% reported that they exercise more than once/week 8/3/2017 8

Expand Options for Community Living • Individuals and Self-Advocates Who Chose Their Home • An average of 33% reported that they made their own choice (25%-46%) • An average of 33% reported that they had some input in choosing (25%-44%) • An average of 33% reported that someone else made the choice 25%-49) 8/3/2017 9
