 Health system in Greece follows the evolution

  • f the Greek state it self

 Whereas in most countries Health systems start

developing just after second world war, Greece comes into the phase of evolution and reform just after1974 – falling of military dictatorship

 Firstly, a serious effort of application of a well

structured health system occurs in 1983, with the establishment of National System of Health (L. 1397/83)

slide-26
SLIDE 26

1er periode 1833 – 1922 :

  • Creation of first insurance services
  • Founding of Hospitals and School of medicine of University of Athens
  • Basic laws fro the organization of the health system

2me periode 1922 – 1945:

  • Financial help (loans) towards the improvement of health and Hygiene

conditions and creation of 35 new temporary hospitals due to incoming of 1,5 million refugees from Eastern (small) Asia.

  • Founding of Ministry of Health, Welfare and Perception(L.2822/1922).
  • Founding of Institution of Social Securities (L.6298/1934).
  • Enactment of preconditions of the organization and function of public

hospitals

3me periode 1945–1974:

  • Adaption of federal politics and social control of health services
  • Founding of Organization of Insurance of Agricultural workers (L.

4169/1961)

  • Establishment of a service in rural area (agriculture) (L. 67/68)
slide-27
SLIDE 27

4me periode 1974 – aujourd’hui, avec 3 parts: (α) period before the founding of NHS :

  • Proposition from a Governmental Centre (Program & Finances): to

create a united National Health service, unification of insurance sectors and social design for covering health needs

  • Plan of Doxiadis: the most important effort to create a modern health

system (never took place due “pressures” from the political- medical lobbies) (b) Founding of N.H.S. L. 1397/83: Founding of Central Council of Health, of National Organization of Drugs and N.H.S

L.1397/83:for the first time the basis for the development of N.H.S takes place

Main characteristics: decentralization, social control, united program, emphasis on Primary Health Care and creation of Health centers (c) Periode apres creation du N.H.S.: Reports foreign and Greek experts (1994), «Plan for the citizen» (2000), introduction of management in hospitals (Ν.2889/2001), Changes in to organization L.3329/2005, 3527/2007 and very recently L. 4238/ 2014 ABOUT Primary H.C.

 Public health sector deals with important problems of

  • rganization, management, quality and efficiency in its

services

 Demographic changes, epidemiology characteristics, new

technologies were managed with laws without substantive business and governmental scheduling

 Health cost refers to hospitals’ cost which is almost the half of

total health expenses

slide-29
SLIDE 29

Main problems are:

Lack of design of national strategy about social security: disintegration of the system causes big problems of financial efficiency. Organization of social security based on the professional group means that fate of many funds depends on that of the status of corresponded occupational groups

Inefficiency of ways of financing health units: existence of hospital deficits due to no correlation between nursing cost/ real cost leads to more state funding that contribute to phenomenon of illegal ways of financing and uncontrolled expenses

Lack of motives to increase productivity and staff in Primary health care units:

  • ver crowded urban centers

Lack of a system of assurance of quality, efficiency and productivity of services
