  2. Smoking cessation Public Health Agreement (PHA) PHA objective: • Provide one to one intensive stop smoking support on a weekly basis over a minimum 6-8 week period. Maximum support for up to 12 weeks • Undertake carbon monoxide (CO) reading • Monitor quit rates at 4 weeks post quit date • Record and submit all client data to Surrey County Council Public Health Team via PharmOutcomes Treatment Outcome Quit at 4 weeks: £60 Not Quit at 4 weeks (quit date set, £25 support delivered): Lost to follow up No payment Pharmacotherapy – Nicotine Replacement Therapy reimbursed at the Drug Tariff price
