The Mission of the Northside Child Health Center is to provide accessible, high-quality, integrated primary health care to children and qualifying adults in the Montrose and Olathe Communities by offering culturally competent Mission & services, and health-related education and outreach. Value Statements We are committed to respecting and valuing every person's unique experiences and background. To provide high quality health care services we must acknowledge and be responsive to the health beliefs, practices and cultural and linguistic needs of our diverse patients.

Our role and place in Montrose Health Pediatric primary care option   Recognized as an Enhanced Primary Care Medical Practice  Certified as a Medical Home  Provide full-scope pediatric medical services Pediatric Safety Net   Partnership with Pediatric Associates ensures that all children have access to pediatric specialty services Integrated service provider   Onsite medical and mental health

  6. Team Based Care/SIM Behavioral Health Integration Prevention Services School- Based Health Center Projects SBIRT, ACEs, AHCM and STEPP and initiatives Vaccines for Children COPE Services STAT evaluation (new) & Autism Echo Project participant

  7. Goal- to improve effectiveness, efficiency and quality of services in the primary care setting Team Based Care/SIM Goal- to improve integration of behavioral health services through practice transformation and quality improvement efforts

  8. Preventive Health Services - SBHC Funded by Colorado Department of Public Health and Environment Year 3 of 5 year grant cycle Achievements measured against nationally recognized quality metrics and performance measures

  9. Vaccines for Children (VFC) Program STARTED AUGUST 2017 INFLUENZA VACCINATION WE HOPE TO EXPAND ON SCHOOL PARTNERSHIP FOR 85 UNIQUE VACCINE VISITS SO CATCH POINTS TO GIVE NEEDS FAR IMMUNIZATIONS TO STUDENTS
