Top Denial Reasons – Quick Checklist

Are you enrolled/revalidated? Did you correctly enter your provider type, specialty and taxonomy and include the 9-digit ZIP code of your service location? Are you billing from the correct provider ID and NPI number? If the answer to any of the above is “no”, your claim will be denied.
