Across the cohorts: ▪ 59 cohort 2 practices reported issues ▪ 35 cohort 3 practices reported issues ▪ 3 CMHCs reported issues By the numbers: ▪ 66 practice sites had issues related to the EHRs or EHR vendor ▪ 20 practice sites had issues related to measure workflows ▪ 26 practice sites had “Other Issues” ▪ Many practices reported multiple issues
