Practice Facilitators and Regional Health Connectors: Guidelines for Practice Outreach

  • Practice Facilitator includes CHITA and RHC on initial email to

set up kick-off meeting

  • Kick-off meetings do not need to be scheduled around RHC

availability

  • If the RHC is unable to attend the kick-off meeting,

coordinate with RHC to plan introduction to practice

  • RHC contact information for each practice should now

appear in SPLIT (SIM Dashboard/SIM Summary/RHC Info)
