GOOD STANDING COHORT 3 ▪ SIM-Only Practices : ▪ Achieve a rating of complete (4) for at least 50% of activities in Building Blocks 1, 2, 3, 4, and 7 on year 1 milestones. The remaining score on those building blocks must be Actively Addressing (3) or Just Started (2). ▪ CPC+ Practices : ▪ Achieve a rating of complete (4) for at least 50% of activities in Building Blocks 1,2,3,4,7,8,9 and 10 of year 1 milestones. The remaining score on those building blocks must be Actively Addressing (3) or Just Started (2).
