  • An additional £2.4bn a year to support general practice

services until 2020/21

  • Ambition to strengthen and redesign general practice
  • Vision built on the potential for transformation in general

– Enabling SELF CARE and direct access to other services – Better use of the talents of the WIDER WORKFORCE – Better use of digital TECHNOLOGY – WORKING AT SCALE across practices to shape capacity – EXTENDED ACCESS TO GENERAL PRACTICE including evening and weekend appointments

What are Primary Care Networks?

  • Groups of general practices working together with a

range of local providers to offer more personalised, coordinated health and social care to their local populations

  • 30-50000 Population
  • All practices expected to sign up
  • Funded by a national Direct Enhanced Service
  • Networks receive recurrent funding to employ

additional staff – Social Prescribers, Pharmacists, Physiotherapists, Physician Associates, and Community Paramedics

  • Each PCN will be led by a clinical director
