What is ‘Intermediate Care’?

  • Not ill enough to be in

acute hospital but not well enough to be home unsupported

  • Most services delivered

in people’s homes but also in beds

  • Need to get the balance

right between home/hospital

  • Keep people out of hospital and in their
  • wn homes, if that’s what they want

So w hy do w e w ant to change?

  • We believe we can provide more care for people at

home and help keep them out of hospital

  • By concentrating our beds onto fewer sites we can do

more with our resources and get better outcomes for patients

  • We believe that our proposals provide better value for

money in the long term for local people

slide-6
SLIDE 6

What are w e proposing?

  • To provide an additional 40 nurses, therapists &

support workers to deliver home based care – an increase of around 33%

  • To re organise our beds into 3 main sites providing

118 beds, with a further 40 beds spread across the county.

  • To provide a specialist unit dedicated to helping

people to rehabilitate after a stroke.

slide-7
SLIDE 7

What is the evidence for this?

  • Over the last 3 years several home based services

across Norfolk have developed very successfully and received very positive feedback from patients & carers.

  • Highest satisfaction ratings & very low level of

complaints

slide-8
SLIDE 8

What is the evidence for this?

  • There are a number of national studies which have

demonstrated the value of a mix of home based care with beds being available for those who need them

  • Studies have shown that having well resourced, trained

staff to provide dedicated rehabilitation get the best

  • utcomes, eg for strokes.
slide-9
SLIDE 9

What is the evidence for this?

  • Clinicians have carried out local audits which have

shown that with some support available for people at home, 7 days a week, we can safely get people out of hospital quicker and use less hospital beds (by around 28-30%)

So w hat are w e consulting on?

  • Strengthening home based services by an additional

40 nurses, therapists and support workers

  • Reducing bed numbers from 201 to 158, with 3 main

centres for NHS beds:

  • Dereham (66 beds)
  • Norwich (26 beds)
  • Either Kelling or N. Walsham (26 beds)
slide-11
SLIDE 11

So w hat are w e consulting on?

  • A further supported 40 beds across the county at a

range of locations

  • Within the beds at Dereham providing a dedicated

Stroke rehab unit for patients who need it.

slide-12
SLIDE 12

So w hat does this mean for community hospitals?

The current beds at Benjamin Court, Fakenham, St Michaels Hospital, Swaffham Hospital, and Ogden Court, Wymondham and either Kelling or N. Walsham would close. Other outpatient & other services would continue at Swaffham, Wymondham, N. Walsham & Kelling St Michaels Hospital and Benjamin Court may close

But………

  • We have committed to explore alternative uses for

some of these sites such as Supported beds, Transitional beds, or Long Term (Continuing) Care
