  14. 6 th Grade Immunizations Immunizations necessary to enter  6th grade Meningococcal  Tdap (tetanus, diphtheria & acellular  pertusis) After 10 th birthday  By first day of school 

Absence Reporting  Please call 908-479-6336 option 1  Excused absence = notify office  MORE than 3 days = physician’s statement  Submit physician authorized excuses and absence excuses to the Health Office  Without documentation = recorded as unexcused  Ten unexcused = referred to the municipal court  No PE = No after-school sports participation  No School = No after school activities

  16. Medication Administration  All medications including over the counter must be given by the school nurse  Signed permission from the parent/ guardian in addition to the physician ’ s order must be completed  The medication must be in the original labeled container
