This form should be used only after a CE has actively attempted to purchase the necessary items to comply with meal plan requirements. The form must not be used for anticipated shortages.

additional questions will automatically populate. Milk – most requested waiver during summer feeding due to manufacturers limiting the types of milk being produced or limitations set in stores

  • Produce vendor shorted order of carrot

sticks; therefore CE will not have enough Red/Orange vegetables this week until the next order is received

  • CE operating CACFP cannot purchase

enough vegetables, fruit, MMA, or grains due to grocery store purchasing limits set

approved for dates that exceed 2 weeks. Provide what meal pattern alternatives you are serving such as serving a different vegetable component or serving more enriched grain products if whole grain rich products are not being served.
