  • Phase one business guidelines:
  • Maintain at least six feet between consumers
  • Fundamental cleaning practices
  • Must avoid any instances in which groups of more than 10 are in one location

  • Make sure to identify specific COVID‐19 tests you will be performing a
  • Include estimated number of tests to administer
  • Medicare Part B
  • 855A for sole proprietorships
  • 855B for anyone else
  • Right now fees are being waived under the 1135 waiver and days to process

are at under 7 days, as compared to average of 3 months before
