DOCUMENT
Submit documentation of disability to ODSS – scan/email, fax, mail Definition of disability: limits one or more major life activities and participation in courses, programs, services, jobs, activities,
and/or facilities
Documentation must be from appropriate medical or mental health professional Psychoeducational or Psychological Evaluation or Re-Eval. (from school psychologist, private
psychologist, psychiatrist, etc.)
Letter from medical professional (doctor, audiologist, etc.) May use one of our forms – contact ODSS to obtain
Must contain recommendations for reasonable accommodations
INITIAL MEETING
Beginning of Interactive Process In-person, Phone, or Zoom Video Chat (need a computer with webcam & we send
link to student)
We review request & discuss with student:
How disability impacts student, Documentation & recommendations, Options, Resources available
DECISION
Reasonable accommodation? Director makes determination of approval, rejection, need more
information
Informs student
PLAN OF ACCOMMODATION
If approved, we collaboratively develop Plan of Accommodation.
Could include (most common):
“Letter of Accommodation”:
STUDENT sends to professors & STUDENT meets with/talks to Prof to implement academic accommodations, with my support
Notification to Housing, Dining, Facilities, etc.
Referral to on- or off-campus offices/agencies (OVR, Counseling, Tutoring, Sight Services, etc.)
Recommendations/Training on Assistive T echnology
Academic Coaching
IN GENERAL – RESOURCES & SUPPORT
