- Metal vehicle gate closed, squashing child between gate and post
- No appropriate sensor safety device fitted

Security company sentenced for generator death failings
- Lone security guard died from carbon monoxide poisoning
- A petrol generator was being used inside a derelict building he was
guarding
- No safe systems of work or adequate lone working procedures.
After the hearing, HSE inspector Melvyn Stancliffe commented: “This was a tragic and completely avoidable death that has devastated his wife, family and friends. Their loss is made worse by the fact he was only covering the night shift as a one off, but sadly never returned home.
