Key Edits • DOH inspection  Homes must meet HUD S ection 8 Housing Quality S tandards • Contracts with Host Home Providers (HHP)  PASAs to create contracts with HHPs and provide copy of contracts to the Department upon request • Colorado Adult Protection background check  To be conducted for those who provide direct care • Critical Incident Reporting (CIR)  Guidance on what qualifies as a CIR, when to report, and how to report 3

Edits from May Feedback • 3-person Host Homes  Language modified to provide clarity on expectations for 3-person Host Homes  Qualifying criteria for 3-person Host Home added to rule • Added language requiring Host Home providers to reside in the Host Home • Modified contract requirements  Revised to state must have contracts in place and available upon request • Added language for oversight during a transition  Who must be present during transition  Ensure during move money and personal items are transferred 4

Refer to page 4, 8.609.5.B.3.b 7
