Date: September 18, 2015 Location: San Mateo, California Recommendation: Not In Policy At approximately 08:00 hours, an off-duty officer discharged a personal firearm inside his home, injuring himself. No one else was injured.

Date: February 5, 2017 Location: Livermore, California Recommendation: Not In Policy At approximately 17:31 hours, an off-duty officer discharged a personal firearm inside his home, injuring himself. No one else was injured.

Status of Open Officer-Involved Shooting Investigations

OIS Investigations Update

The 4th Quarter 2016 OIS Investigations Report was presented to the Police Commission on February 1, 2017. Since that report:

One OIS investigation opened (OIS 17-002) on February 5, 2017. Two completed OIS investigations (OIS 15-006 and OIS 17-002) were presented to the FDRB on March 29, 2017. One OIS criminal investigation concluded (OIS 15-003) on April 12, 2017. Tolling per 3304 CA Govt. Code is over.

Open OIS Cases Summary

As of April 18, 2017, there are fifteen (15) open SFPD OIS investigations:

One (1) charging decision has been received from the Office of the District Attorney of San Francisco. This case is being prepared for presentation to the next Firearm Discharge Review Board. Eleven (11) criminal investigations are under review by the Office of the District Attorney of San Francisco (14-005, 14-006, 14-007, 15-001, 15-002, 15-003, 15-007, 15-008, 15-009, 15-010, 16-001, and 16-002). Three (3) criminal investigations are active with the SFPD Homicide Detail, ME, and/or the DA (15-004, 16-003 and 17-001).
