Officer Involved Discharge 004-16

Date: August 19, 2016 Location: Tracy, California Finding: Not In Policy/Training An off-duty officer unintentionally discharged a personal firearm while loading the weapon inside his home residence. The round passed through a bedroom window; no one was injured as a result of the discharge. .

Officer Involved Discharge 005-16

Date: September 26, 2016 Location: 100 Block of Britton Street Finding: Not In Policy During a high-risk search warrant service, and officer assigned to the Tactical Division unintentionally discharged his Department-issued handgun while attempting to activate a weapon-mounted flashlight. The bullet lodged in an unoccupied closet; no one was injured by the discharge. .

ICD 12-0664

Date: November 14, 2012 Location: Fulton Street and 47th Avenue Finding: In Policy Cause of Death: Acute Drug Intoxication An officer on patrol encountered a person near Golden Gate Park who appeared to be under the influence of a controlled substance. The officer attempted to detain the subject, but he fled on Fulton Street, and the officer chased him. The officer caught up to the subject at Fulton and 47th, as additional units responded to assist the officer. The subject physically struggled with the officers for several minutes, until he was placed into

  • custody. The subject appeared to be in an altered mental state, and an

ambulance was summoned. When the subject was placed in the ambulance, he went into cardiac arrest. He was transported to St. Mary’s Hospital and expired on November 30, 2012.

ICD 14-0183

Date: October 10, 2014 Location: 1100 Block of Larkin Street Finding: In Policy Cause of Death: Lethal Cardiac Arrhythmia A psychiatrist called police to request a 5150 evaluation of his patient at the same time 911 received calls regarding a naked, bloody man acting irrationally inside an apartment lobby. Two two-officer units responded and encountered the naked, bloody subject in his open apartment. The

  • fficers called for paramedics and restrained the subject, who continued

to physically struggle against the restraints. Paramedics arrived and administered a sedative to the subject, who went into cardiac arrest. He was transported to SFGH, where he was pronounced dead.

OIS Investigations Update

The 3rd Quarter OIS Investigations Report was presented to the Police Commission on January 4, 2017. Since that report: One OIS investigation (OIS 17-001) was initiated on January 26, 2017.

Open OIS Cases Summary

As of January 24, 2017, there are sixteen (16) open SFPD OIS investigations: Thirteen (13) criminal investigations have been submitted by SFPD criminal investigators and are awaiting charging decisions by the Office of the District Attorney of San Francisco (14-005, 14-006, 14-007, 15-001, 15-002, 15-003, 15- 004, 15-007, 15-008, 15-009, 15-010, 16-001, and 16-002). Two (2) criminal investigations are have active investigative pieces with the SFPD Homicide Detail, ME, and/or the DA (16-003 and 17-001). The corresponding SFPD administrative investigations are tolling. One (1) investigation is administratively tolling because of injuries sustained by the officer involved (15-006).
