Devised and introduced a HES Feedback Form (HFF) Added a diabetic flag to each patients PAS record Set up a weekly activity report (including discharges) One stop laser clinics Urgent DR patients are telephoned

If the patient has R3s and has been discharged back to DES, we know they require an appt within two months of discharge If the retina has not been examined, we know that a DES

  • appt. is required.

(if the patient is suitable) This information enables us to ensure the patient record reflects any treatment or additional follow-ups in HES
