  1. Damage and Loss Assessment Methodology Health Sector Exercise J. Roberto Jovel

  2. Contents 1. Background Information on Gujarat Earthquake and on Kachchh District 2. Description of Exercise 080808 RJovel 2

  3. Background Information The Gujarat Earthquake In February 2001

  4. Location of Study Area Kachchh District 080808 RJovel 4

  5. The February 2001 Earthquake in Gujarat Extensive damage and destruction in urban areas; minor destruction in rural areas In Health Sector, widespread damage and destruction of medical facilities, equipment and supplies The following were destroyed Number - Health centers 22 - Sub-centers 105 - Dispensaries 15 - Anganwadys 192 - Total 334 080808 RJovel 5

  6. The February 2001 Earthquake in Gujarat.. Value of damage estimated as Rs 220 Crore (US$ 47 million) No losses were estimated at the time, that would have included the cost of treatment of (physically and psychologically) injured persons as well as the cost of monitoring and controlling outbreaks of diseases 080808 RJovel 6

  7. Occurrence of Malaria After Quake The Gujarat State Department of Health and Family Welfare reported an increased incidence of malaria cases after the disaster, due to • An increased breeding of the vector – Higher storage of drinking water due to irregular water supply – Many abandoned, partially-filled water tanks 080808 RJovel 7

  8. Occurrence of Malaria After Quake.. • Occurrence of just above normal rainfall rates after disaster • Increassed human-vector contacts • Population displacement after earthquake • Labor migration from malaria-endemic regions 080808 RJovel 8

  9. Malaria Incidence in Kachchh After Earthquake 4000 3500 3000 Number of Cases 2500 2000 1500 1000 Earthquake 500 0 Jan 2001 Jan 2002 Apri Jul Apri Oct Plasmodium Vivax Plasmodium Falciparum 080808 RJovel 9

  10. The Exercise Scope of Work

  11. Scope of Exercise to be Done Workshop participants are requested to estimate the value of losses sustained after the earthquake due to the higher incidence of malaria in Kachchh District, brought about from neighboring areas where the disease is endemic, including – The cost of treatment for malaria patients, and – The cost of vector control to reduce malaria incidence to normal levels 080808 RJovel 11

  12. Assessment Process Number of Patients Population at Risk Arising from ‘Quake In Affected Area Unit Treatment Unit Costs for Costs Vector Control Medical Treatment Vector Control Costs = Losses Costs = Losses Total Losses: Medical Treatment + Vector Control 080808 RJovel 12

  13. Steps for the Assessment 1. Define baseline for assessment – Long term trends – Monthly variation of malaria incidence 2. Define number of cases due to disaster – Plasmodium Vivax – Plasmodium Falciparum 3. Determine unit costs for treatment – Outpatients – In hospital 4. Estimate total cost of treatment 080808 RJovel 13

  14. Long-Term Malaria Incidence in Kachchh 450000 400000 350000 300000 Number of Cases 250000 200000 150000 100000 50000 0 1985 1988 1991 1994 1997 2000 P. Vivax P.Falciparum 080808 RJovel 14

  15. Number of Malaria Cases in Kachchh during a normal year 450 400 350 Number of Cases 300 250 200 150 100 50 0 Jan Feb Mar Apr May Jun Jul Aug Sep Oct Nov Dec P. Vivax P. Falciparum 080808 RJovel 15

  16. Steps for the Assessment .. 5. Estimate losses due to vector control 6. Determine number of inhabitants in affected area – Urban areas – Rural areas 7. Determine unit costs for vector control – Urban areas – Rural areas 8. Estimate cost of vector control 9. Estimate total losses = treatment of patients + vector control 10. Compare value of estimated total losses versus value of damage 080808 RJovel 16

  17. Vector Control Spraying Residual Spraying Fog Spraying 080808 RJovel 17
