Year 1 Programme Structure

On successful completion of Level 4 (year 1) students will have gained 120 credits by passing the above modules .

Programme Structure Year 2

On successful completion of Level 5 (Year 2) students have gained another 120 credits by passing the modules. Students will then have 240 credits in total and will be awarded the FdSc Health & Social Care Practice.

Practice Mentor /Supervisor

The Practice Mentor / Supervisor will be a registered health or social care professional within the student’s area of work Students undertaking the Foundation Degree must be supported in practice by a Practice Mentor (Competency Route & Practice Supervisor (Non-Competency Route) A Practice Mentor / Supervisor must be professionally competent and be able to make assessment decisions in practice; verifying the work based student’s competencies (competency route) or practice experiences (Non-competency route).

Qualifications for Practice Mentors Supervisors

  • Practice Mentors / Supervisors must be professionally

competent i.e. they must have a minimum of one year’s post-registration experience in their field of

  • practice. In addition to aid their role as mentor /

supervisor they will be expected to have participated in an FdSc mentor /supervisor workshop.

  • These are held at both campuses Teesside &

Darlington

  • Dates are included on the Practice Support Site

The Practice Mentor / Supervisor will:

  • Provide a patient/client-centred approach to

caring/service provision in the clinical setting in which student learning can be negotiated and facilitated.

  • Work/provide supervision with/for the

student on a regular basis.

  • Ensure that a programme of learning is

identified and implemented for the student.

  • Keep the Academic Tutor/s and Clinical Manager

informed of the student’s progress & development as appropriate.

  • Record information of the student’s progress and

development within the student’s e-portfolio.

  • Provide on-going feedback to the student.
  • Make a final judgement regarding whether the

student has achieved the practice experiences (non-competency route) & competencies and benchmarks (competency route).

Agendas for each review

  • Practice Mentor’s / Supervisors can access the

agenda for each review in the e-portfolio at the top of the review form.

  • After each review the Student, the Practice

Mentor /Supervisor and Academic Tutor complete their specific section identified on the review form.

  • If the Practice Mentor/ Supervisor has any issues,

concerns or queries they should contact the Academic Tutor identified to support the Student and Practice Mentor / Supervisor.

Informed Consent Declaration forms

  • If specific patient/client data/information from the

practice area is used within the student’s work an informed consent form must be included. The Practice Mentor/Supervisor is responsible for:

  • Providing the student with appropriate support and

guidance whilst obtaining informed consent form the patient/client and/or patient/client representative.

  • Signing the Obtained Informed Consent Declaration

Form, only if they are confident that the criteria have been met.

Essential Evidence Requirements

  • Each competency identifies the essential evidence

required.

  • The student needs to be observed undertaking safe

practice whilst demonstrating the appropriate attitude and professional conduct.

  • This involves direct observation of the student’s

practice skills, abilities, attitude and conduct by the Practice Mentor. Evidence that the student’s practice is safe will be provided by the Practice Mentor who will assess the student and determine their ability to meet the practice competencies.

  • The Practice Mentor / Supervisor will verify the

student’s competence / Practice Experience as they progress through the academic year

  • The Practice Mentor/ Supervisor will confirm the

student’s overall competence as being achieved at the end of the academic year.

  • Observation of safe practice and appropriate

attitude/professional conduct by the Practice Mentor Supervisor.

  • Discussion through question and answer with the

Practice Mentor/ Supervisor to establish the student’s understanding of the underpinning knowledge and its application to practice.

Reflection

  • Throughout the module students will introduced

to the concept of reflection & becoming a Reflective Practitioner.

  • In the e-portfolio students will have access to a

template for reflective activities enabling them to capture meaningful learning experiences.

  • Students are encouraged to develop the skills to

reflect & learn to enhance their practice.

Testimony of Others

  • Where the student’s Practice Mentor Supervisor is

unable to observe the Student witness testimonies to support the achievement of competence may be sought.

  • The testimony must be relevant and specific to the

student’s experience and provide supporting evidence of their achievement of practice benchmarks / competencies / practice experiences.

  • The Practice Mentor / Supervisor must agree to the

Student seeking testimonies & where deemed appropriate these must include the title & signature

  • f the witness.

Essential Evidence

  • In week 23 of the programme the students

will be required to submit a 3000 word reflective care study. The Module Tutor and Academic Tutor will support the students in preparation for this.
