Derrick & Well Ops Incidents • Stabbing joint of 5-1/2” DP (335kg), elevators opened & joint fell 1m to rotary table, tipping towards set-back area • Breaking out riser bolts at rotary table, bolt (20.7kg) dropped, falling between cover plates 7.8m to mezzanine deck • Joint of 5” DP (275kg) fell 1.2m from elevators to rotary table; elevators not securely closed • Using chain hoist to install ESP motor in completion, chain parted when blocks raised; hook & chain (2.3kg) fell 9m to rig floor • Breaking out stand from saver sub, die from top-drive grab (0.4kg) fell 30m to rig floor

Crane & Lifting Operations • Section of deluge spray pipe-work (32kg) detached from crane “A” frame ladder, falling 17m to main deck • 2” castellated nut (0.7kg) from main rope termination fell 50m from crane boom to pipe deck • Striker plate slid down crane hoist, struck main block & parted with piece (2.4kg) falling 20m to skid deck • Lifting BOP skid cylinder assembly, sling parted & cylinder fell into sea • Lifting bundle of scaffold tubes, load struck platform; 31 tubes fell 15 m to landing area

Other Dropped Objects • On maintenance visit to NUI, spotlight (3kg) found missing - fallen into sea • Steel plate (1.3kg) ejected from waste heat unit, fell 20m to helideck • Preparing to lower bunkering hose, rubber bumper protector detached from hose station, fell 35m to Level 1. • Section of corroded unistrut bracket (2kg) for cable tray fell 5m • Sheet metal (8.6kg) from emergency generator ducting parted and fell 9m to walkway • Valve (22kg) detached from 4” drain & fell 0.9m onto production pipework in cellar • Section of steel grating (1.5kg) dislodged from gas treatment module deck to sea (8m), striking pipework

Fittings • Part of corroded steel bracket (6kg) found to have fallen 6m to main deck • Air regulator (6.4kg) from hose fell 5m from upper deck to main deck, striking IP on knee • Chock (7.8kg) centralising conductor in well bay fell into sea • Door handle wheel (5k) detached from hydraulic hatch & fell 9m down port column

Unsecured • Scaffold tube (10.7kg) dislodged during construction, falling 7m to cellar deck • During construction of scaffolding, section of metal (10.3kg) fell 3.3m to deck below • End of mud jumper hose separated from temporary fastening & fell 9m from cantilever to main deck • Rubbish, including bin lid, blown overboard by helicopter down- draft • Scaffold pole (6kg) found to have fallen from rack on skid deck (2.5m) to walkway • Bag of nuts & bolts (10kg) fell off pallet being moved; bag fell 22.4m from mezzanine to main deck • Box of 5kg gym plates holding open accommodation door; box split; two plates fell through grating 3m to walkway

Personal • Spanner dropped through deck penetration, 15m to walkway below • Rope access team dropped tool (0,5kg) from monkey board 25m to rig floor • Removing cover from rig floor opening, bushing (1.6kg) dropped 18.3m to mezzanine • Operator dropped socket set 13.5m from crane engine-room; case & sockets fell further 7.5m (wind & oily gloves)
