  • Running ESP cables, sheave hinge roller (4.5kg) fell 12m to rig floor
  • Down-hole gauge carrier (142kg & 5.5m) detached from wire-line &

toppled to deck

  • During recalibration of hydrocracker (sic) and encoder plastic gear (0.5kg)

fell 30m to rig floor

  • Link-tilt crank to bale pin (0.45kg) backed out and fell 30m to rig floor
  • Top drive struck cross-member of dolly rails; bolts failed & cross-member

(40kg) fell 3m to rig floor

  • Steel tube (2.9kg) fell 4.5m from rig floor during operations
  • Removing cap (3.5kg) from test tree actuator, floorman dropped cap 10m

to rig floor

  • Picking up joint of 7” tubing with 7 5/8” elevators, it slipped out and slid

back to cat-walk

  • Rigging up wire-line, line parted & fell 21m to drill floor when sheave

raised

  • During high winds light fixture bracing (0.9kg) detached & fell

10m to rotary table

  • During very high winds section of cable rack detached & fell to rig

floor

  • Descending top drive contacted tugger line (1.05kg/m); both ends

fell to rig floor; sheave at 44m

  • Section of drive track (28.6kg) detached from vertical pipe

handler & fell 34.4m to doghouse to rig floor

  • Fabric bag containing DP protectors (140kg) detached from

crane hook & fell 4.6m to deck

  • Starting crane, it slewed dislodging chock (80kg) from rest;

chock fell 8m onto pipework

  • Installing jacket extension, tubular piece (15 Tonne) detached

from lifting frame and fell to seabed. (Bolted flange failure)

  • Moving diverter running tool (1100kg) to catwalk, sling broke &

tool fell 2m to pipe deck

  • Crane rope guide failed; pieces (1.1kg & 0.6kg) fell 6,9m to

walkway

  • Sheet steel cover on palfinger crane hydraulics detached in high

wind & fell to pipe deck

  • Lowering container (9.8 tonne), it was dropped 3.4m onto timber

deck beams and well test pipe-work

  • Removing 12” spool, it slipped in rigging and dropped 1.5m,

striking scaffold

  • Lowering block & booming out, headache ball struck sheave;

whip-line parted; headache ball landed on deck of supply vessel

  • Using crane to free brine hose tangled with potable water hose,

strop failed and hose fell 9m striking member of deck crew

  • 3m stinger found to have detached from whip line hook and

assumed to have fallen into sea 2015

  • During crane ops, crane rope struck gas detector reflector plate

(0.5kg) which fell 30m to scaffolding

  • Sheath protection pin (0.6kg) from crane main block sheave

dislodged & fell 44m to main deck

  • During jarring, section of passive fire protection (6kg) detached & fell

4.2m onto scaffold structure

  • Section of corroded scaffold tube (2.9kg) found on walkway; believed to

have fallen 6m

  • Length of corroded grating (1.2kg) fell 19m from walkway to cellar deck
  • Angle iron bar (3.5kg) found on walkway at bottom of flare stack after bad

weather

  • Piece of cladding found at deck level between two exhaust stacks after

adverse weather

  • Scaffold pole (4.5kg) fell 5m to deck in adverse weather
  • Redundant lighting bracket (2.2kg) detached & fell 14.5m to level 1.
  • Sign (7kg) detached in adverse weather and fell 7m onto top of container
  • During high wind section of cladding (13.2kg) detached & fell 7m to

walkway, striking pipework on way down

  • Metal blank plate (14.5kg) fell 11m from accommodation module to

walkway

  • During high winds section of fibre-glass pipe lagging fell from

pipe rack to walkway

  • During visit to NUI section of derrick cladding found to have fallen

into skip. Believed due to high winds

  • After high winds section of cladding from accommodation

missing, believed fallen into sea.

  • During high winds piece of uni-strut and perspex (8.25kg) fell

3.6m to walkway

  • Removing junction box (9.1kg), it slipped out of supporting rope &

fell 5m to walkway

  • Pin (1kg) from shackle from gangway cone securing strap found

fallen 12.4m to main deck

  • Bundle of instrument tubing (4kg) dropped through grating & fell

4m striking individual on shoulder-blade
