About the National Center for Fatality Review and Prevention • The National Center for Fatality Review and Prevention (NCFRP) is a resource and data center that supports child death review (CDR) and fetal and infant mortality review (FIMR) programs around the country. • It is funded in part by Cooperative Agreement Number UG7MC28482 from the U.S. Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA), Maternal and Child Health Bureau (MCHB).

HRSA’s Overall Vision for NCFRP • Through delivery of data, training, and technical support, NCFRP will assist state and community programs in: – Understanding how CDR and FIMR reviews can be used to address issues related to adverse maternal, infant, child, and adolescent outcomes – Improving the quality and effectiveness of CDR/FIMR processes – Increasing the availability and use of data to inform prevention efforts and for national dissemination • Ultimate Goal: – Improving systems of care and outcomes for mothers, infants, children, and families

  7. Why Version 5.0? Why the New Name for the System? Question Changes: • Addition of Fetal and Infant Mortality Review (FIMR) questions (3 sections which CDR users WON’T see) • Section A (Child Information) re-organized • Re-vamped reporting of child abuse, neglect, poor supervision and exposure to hazards • Suicide risk factors now in standalone section • Streamlined data entry: new skip patterns if infant never left hospital following birth

Rollout Plan & Key Dates • Version 5 will be launched Monday, April 23 rd • System will be unavailable beginning Monday, April 16th - Sunday, April 22 nd • All cases entered with previous Versions will still be available; new questions will be blank for existing cases • No new system requirements needed for upgrade • Free!! • Please contact your state CDR coordinator to request paper forms

  10. How Do I Login to Version 5.0? • Web address is changing to https://data.ncfrp.org on April 23 rd – A web re-direct will be put in place if you forget – Good idea to update bookmark, if applicable • Passwords and User names are NOT changing if you currently have a login to the CDR-CRS

  11. Data Analysts: Data Codebook Updated! • The re-organization of Section A (Child Information) and old Section I (Act of Omission/Commission) has caused the re-naming of many variables in Version 5 • Contact NCFRP if you would like a Version 5 Data Codebook in advance of the release date
