  4. WHAT IS ASSISTIVE TECHNOLOGY? In general, the term means any item, piece of equipment or product system, whether acquired commercially off the shelf, modified or customized, that is used to increase, maintain, or improve functional capabilities of a child with a disability. 4

  5. AS ASSIST SISTIVE IVE TE TECHN CHNOL OLOGY GY REFERR REF ERRAL AL PROC PROCESS ESS • An IEP team must meet in order to consider a student’s need for assistive technology. The discussion about the student’s needs should be documented in the IEP notes. • The following three items ARE required in order for an evaluation or consultation to be completed ➢ AT request ➢ Student Information Form ➢ AT Considerations Checklist 5
