It is the Co-operation of the core components that defines the system.

- The Help Me Grow (HMG) system is designed
to help states and communities leverage existing resources to ensure that communities identify vulnerable children early, link families to community based services, and empower families to support their children’s healthy development through the implementation of four Core Components:
- Child Health Care Provider Outreach,
- Family and Community Outreach,
- Centralized Call Center, and
- Data Collection.

Example of family’s call to MOMS Helpline
- A grandmother calls the helpline with concerns
about her two year old grandson stating that he is not speaking well and his behavior is getting worse.

Why does Indiana need Help Me Grow?
As a state the average age of diagnosed developmental disability in children is older than 3 years old. We make referrals to services, but do not know if the referred services are effective for the child and family. We must do better!

Help Me Grow system
1)Assists families, primary care providers, and other community-based providers including home visiting services in screening for developmental or behavioral concerns in children using the Ages and Stages Questionnaire (ASQ), 2)Statewide resource directory with a feedback loop to identify gaps in services or a wait list for those services, 3)MOMS Helpline is the centralized call center to connect families with specialized programs and services focused on their needs using Help Me Grow care coordinators.

Indiana vision to use existing resources
- Due to the ELAC early support, they will add HMG to
their existing work groups.
- The ELAC Child Development and Well Being work
group is the HMG Leadership Committee.
- The ELAC Data Coordination and Systems Integration
work group is the HMG Data Collection work group.
- Both Project LAUNCH State team and local ECCS
team are the Family and Community Outreach work group.
Help Me Grow Indiana Organizational Chart
Implementing entity – Final implementation authorization, funding direction, federal reporting, presentation approval, and data approval decisions are made by this team. IN HMG Program Manager oversees Leadership Team and communicates to IN HMG Organizing Entity activities and recommendations. IN HMG Workgroups implemented within existing early childhood system entities where possible. These workgroups serve as special project workgroups and subject matter advisory entities. IN HMG Leadership Team reviews activities and recommendations from Core Component committees and provides report and recommendation to Organizing Entity via IN HMG Program Manager. IN HMG Leadership Team is a function within larger existing entity of [to be confirmed] workgroup

Emphasizes incremental improvements over time that continue to produce the best outcomes for children and families, maximize efficiency, and yield best practices that can be applied across the state

What will HMG implementation look like?
- Piloting in the nine MIECHV counties and ECCS
community.
- Marion County including the ECCS Near East side.
- Delaware, Elkhart, Grant, Lake, LaPorte, Madison, St.
Joseph, Scott Counties
- September 2018 first call to the MOMS Helpline
using the HMG model.
Pilot Sites
Help Me Grow Indiana will be a single centralized phone system embedded in MOMS Helpline. Resources, referrals and follow-up / feedback loop will be targeted and developed in Pilot sites.
