Optimising Patient Flows / Care Coordination Helping Health & Social Care to Minimise DTOCs Portal Overview • CHLBS was built by NHS NECS in collaboration with NHS England and North region Social Care colleagues • NECS is NHS Digital IG Toolkit Level 2 compliant • We can build an API to interface with any other information system • Powerful DTOC Analytics and DTOC Benchmarking by NHS Trust and Local Authority • Accessible from any Mobile Device (smartphone, iPad, laptop etc). • It’s simple & intuitive – Carehomes take less than a minute to update their bed state – whenever they want / 24-7 – No formal training required – Registration takes 30 seconds • Commitment to continuous improvement & product development – Monthly User Research informs development priorities – ‘Home Care’ Module coming soon to help find domiciliary care packages – Reporting soon to be optimised to convert into equivalent DTOC costs – NHSE Programme Board has oversight of progress

Optimising Patient Flows / Care Coordination Helping Health & Social Care to Minimise DTOCs Coverage • Extensive North Region Coverage – and Gaining Traction each week: • 3500 registered users • 1320 carehomes have registered and are sharing their live bed state • Representing 55 CCGs / 50 LAs across England • 17 CCGs have already implemented the Live Bed State across the North • Including South Yorkshire & Bassetlaw STP, West Yorkshire STP • Plus, 10 more CCGs are currently being mobilised • And, 3 more CCGs will have mobilised by end October 18 • 33 North CCGs in Total out of 62 (53%) • Plan to eventually link this to the UEC app • Outside of the North region • NHS and Local Authority customers currently mobilising in the Midlands & East and South Regions including: • Integrated Care Systems, CCGs and Local Authorities • Our rapidly expanding customer base makes this the ideal solution to support repatriation and expatriation of patients 5
