technology and frontloading of touchpoints in the first 14 days of care. Purpose: v Decompress Hospital Beds and ED bays v Prevent Admission/ Readmission for observation level and lower acuity inpatient level care. v Increase confidence of providers to send patients home with the support needed to be successful.
