Final Forms

• Final Forms is an online form system, in which you can sign up to play sports

and also complete your emergency medical forms.

• Fill it out once, and your information stays until 12th grade.
• Find information on how to sign up on table out front, or under parent forms

Physical

• In order to participate in an OHSAA (Ohio High School Athletic Association)

sport, you must have a current physical.

• The physical expires once every calendar year (May 2018-May 2019)
• You can find this physical under PPE Physical form on Final Forms once you

have completed all of the emergency medical information.

Pay to Participate

• Pay to Participate fee is 80 dollars per sport.
• You can choose to do a half payment plan, with half due before season, and

half due at halfway point of season.

• If you are on Free/Reduced Lunch, there are exceptions:

○ Reduced lunch gets 50% off a sport. ○ Free Lunch gets FIRST sport free of charge.

• You can pay in one of three ways:

○ Check to Child’s School ○ Cash to Child’s School ○ My School Bucks
