MISSION The Aphasia Centre
dedicated to facilitating the inclusion of people with aphasia in family and community life by bridging the communication gap.

aphasia (from very severe to very mild) 18 + years English, French, best efforts with other languages independent for washroom or attendant/family physically able to attend at 2081 Merivale (SW re transportation) all socio-economic backgrounds (sliding scale) all types of family support or no family support
Home visit (SW) Tour of ACO (S-LP or CDA) Transition group (4 weeks)
a transition group is a 4 week program (2 hours a week for 4 weeks) designed to help the person with aphasia and a family member transition into Aphasia Centre of Ottawa programs and have access to services the group consists of a S-LP, a SW, and 2 to 4 couples/dyads goals: answer questions; identify those who may benefit from individual/couple counselling, S-LP models communication support, identify best program
facilitated by a staff member (CDA or S-LP , sometimes SW); often assistance from volunteers or students 3-5 people with aphasia in a group multimodal approach, communication support natural, conversational approach, with ongoing assessment in context (CIRSAG) group speech-language therapy (Roberta Elman) peer support; social work support as needed

2 SLPs and 1SW who are fluent in French French programs (communicatio n group, physio) Counselling
